Low Ferritin & Fatigue: Why Your Labs Say You're "Fine"

If you're a woman who's been told your bloodwork looks "fine," but you're still dragging yourself through the day, losing more hair in the shower than you'd like to admit, tossing and turning at night, and feeling winded walking up a flight of stairs you used to take without thinking — you are not imagining it, and you are not alone.

One of the most commonly overlooked culprits behind these symptoms is low ferritin — and it's a story I hear from patients almost every week.

Ferritin vs. Hemoglobin: Why "Normal" Doesn't Mean Optimal

Most standard bloodwork checks hemoglobin as the main marker for anemia. If your hemoglobin is within range, many doctors will tell you your iron status is fine and move on.

But hemoglobin only tells part of the story.

Ferritin is the protein that stores iron in your body — think of it as your iron savings account, separate from the iron currently circulating and being used (which is more what hemoglobin reflects). You can have completely normal hemoglobin while your ferritin — your reserves — are quietly running on empty.

This matters because your body pulls from ferritin stores long before hemoglobin ever drops. By the time hemoglobin is affected, you've often been symptomatic for months, sometimes years.

Why the Lab Range Might Be Misleading You

Here's something many patients don't realize: standard lab reference ranges for ferritin are wide, and "normal" is not the same as "optimal."

Many labs list the low end of normal ferritin somewhere around 10-15 ng/mL. But research on iron-related symptoms consistently shows that many women don't start feeling like themselves again until ferritin is well above that floor — often in a higher range that's still technically "normal" by lab standards, but far from ideal for energy, hair growth, and cognitive function.

In other words: your ferritin can be a number that doesn't flag as abnormal on paper, and you can still be significantly iron-deficient in a way that affects how you feel every single day.

The Symptoms That Often Get Dismissed

Low ferritin has a way of showing up in symptoms that get written off as stress, aging, or "just life" — especially for women juggling careers, family, and everything in between:

  • Persistent fatigue that doesn't improve with sleep
  • Hair thinning or shedding, sometimes months after the ferritin drop (iron deficiency hair loss is often delayed, which makes it easy to miss the connection)
  • Poor or unrefreshing sleep, including restless legs at night
  • Activity intolerance — feeling disproportionately winded or exhausted during exercise or exertion that used to feel easy
  • Brain fog, difficulty concentrating
  • Cold hands and feet
  • Brittle nails
  • Irritability or low mood

If several of these sound familiar and you've been told your labs are "fine," it may simply be that the right marker wasn't checked — or the result wasn't interpreted with your symptoms in mind.

Why This Gets Missed So Often

It's not that your doctor doesn't care — it's usually a combination of:

  1. Ferritin isn't always included in a standard CBC or basic metabolic panel. It has to be specifically ordered.
  2. Reference ranges are population-based, not symptom-based — they're built to flag disease, not to define where you feel your best.
  3. Time constraints in a typical appointment make it easy to see a "normal" result and move on without connecting it to your symptoms.

This is exactly why context matters so much in interpreting labs — a number without your story attached only tells half the picture.

What You Can Do

If this sounds like you, here's where to start:

  • Ask specifically for a ferritin level to be included in your next bloodwork, if it hasn't been checked recently.
  • Bring your symptoms to the appointment, not just the lab printout. Fatigue, hair loss, poor sleep, and exercise intolerance are clinically relevant data points, even if they don't show up on a lab slip.
  • Ask what your actual ferritin number is — not just whether it falls in the "normal" range.
  • Talk to your doctor about the underlying cause. Low ferritin isn't a diagnosis in itself — heavy periods, GI issues, diet, and other factors can all play a role, and addressing the root cause matters as much as the number.

You Deserve to Feel Well, Not Just "Not Sick"

There's a real difference between labs that rule out disease and labs that explain why you feel the way you do. If you've been told everything looks fine but you don't feel fine, it's worth a closer look.
